The Chinese General Hospital and Medical Center (CGHMC) successfully performed a transhepatic stent-assisted portal vein aneurysm coil embolization, becoming the first hospital in the Philippines to achieve the significant advancement in medical treatment.
The procedure was performed on a 69-year-old female patient suffering from obstructive jaundice caused by a portal vein aneurysm.
Imaging studies, including MRCP and MDCT, revealed a 3.7 x 4.0 x 3.7 cm saccular dilatation of the proximal main portal vein, which was compressing the common bile duct.
Prior to the embolization, the patient underwent an endoscopic retrograde cholangiopancreatography (ERCP) with bile duct stent insertion, performed by Dr. Michael Louie Lim, a gastroenterologist at CGHMC.
The procedure involved accessing the portal vein through the liver via a small puncture in the abdominal wall.
A metal stent was deployed to protect the vein, followed by the placement of detachable coils inside the aneurysm to prevent further enlargement or rupture.
The procedure was executed by a multidisciplinary team of experts from CGHMC’s Interventional Radiology, Hepato-Pancreato-Biliary (HPB) Surgery, Gastroenterology, Anesthesia, Thoracic and Cardiovascular Surgery (TCVS), Endocrinology and Cardiology Services.
The lead interventional radiologist, Dr. Francis Basilio, spearheaded the innovative technique, supported by Dr. Noruel Gerard Salvador (HPB surgeon), Dr. Kristine Mae Tan (anesthesiologist), Dr. Rudolf Kuhn and Dr. Joemel Francisco (interventional radiologists) as well as a host of specialists from various departments, including Dr. Melissa Uy (endocrinologist), who managed the patient’s hypothyroidism and diabetes mellitus and Dr. Stewart Santos (TCVS), Dr. Kent Tan (interventional cardiologist) and Dr. Billy James Uy (HPB surgeon).
The complex, minimally invasive procedure marks a significant achievement in the field of interventional radiology and hepatobiliary surgery.
